Care Coach:
- Support and mentor individuals to achieve their personal goals.
- Develop and implement personalised care plans.
- Foster a supportive and encouraging environment.
- Lead and motivate a team of support professionals.
- Ensure high standards of care and compliance with policies.
- Coordinate team activities and manage daily operations.
