ACP Elderly Virtual ward / Front door frailty

Delivering admission avoidance of frail older people in the community, preventing hospital admission, frailty in reach and to support the development of a front door frailty same day assessment unit. Supporting frail older patients return home (discharge to assess) The successful post holder will work as a dynamic and proactive ACP within our very successful and developing Integrated Frailty service. The service operates over 7 days and provides the frail population of Bradford the benefit of a holistic Comprehensive Geriatric Assessment. You will be based at St Lukes Hospital. As a team member you will be part of a friendly, supportive, and professional integrated team which comprises of Geriatricians, Nurses, Occupational Therapists, Physiotherapist, Co-ordinators, Advanced Clinical Practitioners, Pharmacists, Social services, and Community services., The post holder will be an autonomous practitioner who will take an active role in responding, stabilising and make safe element of acute care pathways by delivery of advanced assessment, and the management and treatment of frail older patients and will also be proactive in admission avoidance, urgent care response. You will support the development of front door frailty/ frailty SDEC.
