Full Job Description

Join our dedicated team in the Northampton Central Community Team, where we are committed to providing high-quality support to vulnerable adults in our community. We work collaboratively within the Local Areas Partnership (LAP) to create a positive impact on the lives of those we serve. West Northants Council uses the "three conversations" model, where appropriate, which re-emphasises focus on people, their strengths, and the networks through the development of community and Inclusion hubs. We are looking for a worker who is passionate about ensuring people have choice and control over their own support needs and focus on the person, their strengths and networks., Our workers act as 'trusted assessors' for defined areas of support plans. They conduct or support individual assessments and reviews of support plans (including risk assessments of both the customer and carer, in line with eligibility criteria to assess the dependency needs of the customer) or facilitate self-assessments, using the appropriate assessment tools and techniques in order to establish clear, relevant, proportionate, cost effective and appropriate options for the individual.

Able to work in a person-centred manner with people that access to an Adult Social Care Worker. You will support a diverse group of individuals aged 16 and over, including those with Autism Spectrum Disorder (ASD), mental health challenges, and physical disabilities. The role will involve promoting independence, enhancing quality of life, and ensuring the well-being of our service users. Our services, identify their strengths, needs and networks to achieve the best outcomes for each person that promote wellbeing. We aim to prevent, reduce, or delay the need for people to access statutory care and support, maximising independence, and wellbeing in a positive manner wherever possible, delivering the best outcomes for local people that is person- centred. Are you willing and able to work collaboratively with a wide range of partners, including community groups and the voluntary sector as well as colleagues in health and other sectors.

  • Have the passion to promote people's strengths and the ability to make a difference in a safe working approach to social work practices. Proactive and can think creatively. Demonstrate problem solving abilities, working alongside people to enable them to find their own solutions. Do you exercise professional curiosity rather than accept things at face value or make decisions based on assumptions. Have up to date knowledge of legislation and demonstrable experience of delivering social care outcomes to people in need with good communication and IT skills. A degree or NVQ 4 or the ability to demonstrate knowledge gained through detailed practice and application for procedures in adult social care. You will be required to drive and have transport.
