Full Job Description

As an experienced and qualified Social Worker, you will be part of a team working across the Staffordshire. The role focuses on conducting comprehensive reviews of the various Supported living schemes for adults aged 18. The Supported Living Review Team is centrally based in Stafford and conducts service reviews across various schemes in Staffordshire. Our service aims to:

  • Ensure the delivery of least restrictive and most cost-effective support models at a scheme level, enabling individuals to achieve their outcomes.
  • Review the quality and affordability of accommodation.
  • Ensure the delivery of quality care and support at a scheme level.
  • We have a range of key performance indicators designed to embed good practice across the service. Working up to 37 hours from Monday to Friday, flexibility in working arrangements can be tailored to suit your needs, offering both professional growth and work-life balance. There will be opportunity to work remotely with an expectation to be office based for induction and ongoing team meetings. This is a fixed term role for 12 months subject to review and evaluation.Main Responsibility You will:
  • assist in the design and delivery of our Supported Living Commissioning Plan through collaborative partnership working.
  • communicate effectively with a range of key stakeholders to achieve positive outcomes.
  • perform reviews to ensure that assessments and support plans accurately represent the support outlined by the service and meet the individuals' outcomes.
  • translate review information into structured documentation to enable the calculation of proportionate support.
  • work closely with commissioners to ensure the best models of support and accommodation across Staffordshire.
  • undertake key statutory functions within the team and contribute to organizational requirements where necessary.
  • conduct a range of tasks as allocated by the responsible manager and in line with the Service Review schedule.
  • work within a multi-disciplinary/professional team to support individuals in achieving their outcomes in the least restrictive manner, using a range of intelligence and tools.

    You'll have:
  • a Professional Social Work qualification (BA in Social Work, DipSW, CQSW)
  • registered with Social Work England
  • experience and understanding of planned personalised support for adults with assessed social care needs
  • knowledge and understanding of supported living arrangements for adult
  • understanding of learning disabilities, mental health, physical disabilities and their impact on individuals, families, and society
  • knowledge of adult physical, intellectual, emotional, and social development and family dynamics
  • experience working as part of a team in developing services and the professional practice of colleagues
  • excellent verbal and non-verbal communication skills and interactions with individuals and families
  • ability to recognise and manage conflicting values and ethical dilemmas in practice, using supervision, team discussion, and challenging assumptions
  • ability to critically reflect on own practice
  • resourceful and resilient, able to operate in a changing environment

  • Salary £37,938 - £42,708 per annum plus £963 Essential Car User Allowance
