Full Job Description
The main purpose of this role is to ensure effective trust wide identification of patients with alcohol use disorder (AUD), to provide face to face patient interactions that nurture the patient partnership and develop a shared person-centred goal, support optimal management during inpatient admissions, and facilitate timely and appropriate discharge into the care of community services as appropriate. In this key role, you will make a difference by providing person-centred care, prompt support, advice, and education to ED and the wards to build knowledge and skills of staff within these settings in the care and treatment of patients for alcohol related presentations and admissions. The post will be based in the Enhanced Care and Support Team at the Royal United Hospitals, within the Vulnerable People Service. You will work closely with the Emergency Department, working with patients at the front door to provide early intervention, and those who are inpatients.,
- Provide expert advice and assistance to health professionals on the management of alcohol and drug related health problems, especially withdrawal.
- Provide help advice and support to patients with significant alcohol and drug related health problems and those at risk from alcohol and drug withdrawal.
- Provide a specialist service in a variety of settings for patients referred with a range of complex needs, having ongoing responsibility for managing your own caseload, including assessment of service users' needs and the planning, provision and evaluation of care and treatment to meet needs as appropriate and based on best available evidence of effective practice.
- Establish and maintain excellent links between emergency department, inpatient, outpatient, and community services.
- To maintain / improve / update robust alcohol withdrawal protocols for use within both inpatient and outpatient settings, working closely with ED.
- Offer brief interventions to individuals with identified alcohol related issues. This will take place within the RUH. You will be offering advice, education, brief therapy and signposting to treatment services.
- Responsible for assessment of care needs, the development, implementation and evaluation of programmes of care, with the multidisciplinary team within the field of alcohol and drug misuse.
