NHS Somerset ICB is seeking an experienced HCPC registered Occupational Therapist to join the busy All Age Continuing Care/CHC team, which is part of the wider Quality and Nursing directorate. You will be an expert practitioner in occupational therapy practice and provide clinical expertise, specialist advice for adults, children and young people with complex needs who require assessment and equipment provision within the NHS Continuing Healthcare (CHC) and Children and Young People Continuing Care (CYP CC) criteria., We offer flexible working from your first day of employment and we also have an agile approach to home / office-based working. Please note however, that you will be required to work from our HQ, Wynford House in Yeovil for 2/3 days per week and that elements of the role may require you to travel to other locations across Somerset, to attend in-person meetings and meet the needs of the service. You will need excellent assessment and interpersonal skills, and be passionate about supporting individuals with long-term complex needs, experience in All Age Continuing Care is desirable but not essential. You will need excellent organisational skills and a sound, knowledge of MS office packages such as Word, Excel, Outlook, Excel etc., alongside excellent written and verbal communication skills both virtually and in person.   • To act as an expert practitioner for the wider AACC team, other occupational therapy staff, other clinical and social care colleagues in respect of the complex clinical needs of adults, children and young people who meet the CHC/CYP CC criteria.
  • Undertake assessments for individuals with an identified Occupational Therapy need including adults, children and young people who live in residential nursing homes and specialist placements both in and out of county as well as for individuals in receipt of Personal Health Budgets (PHBs) in their own homes and supported living accommodation across Somerset. Make decisions on NHS Continuing Healthcare eligibility for adults as part of a Multi-Disciplinary Team (MDT) approach
  • To be able to move and handle adults, children and young people with complex physical problems and disabilities, assess, prescribe and adjust specialist equipment., NHS Somerset Integrated Care Board (ICB), known as NHS Somerset is the statutory NHS organisation responsible for implementing a health and care strategy developed by the Integrated Care Partnership.
