Full Job Description

Key Results To provide and monitor safe, person-centred care to a designated group of patients, service users and families in a range of care settings under the direction of a Registered Nurse, without direct supervision in line with an agreed plan of care. To support the registered nurse and contribute to the on-going assessment and care planning of patients and service users to improve and maintain their mental, physical, behavioural health and wellbeing. To act in the best interests of patients and service users to promote and prevent ill health and contribute to their integrated care. To monitor the condition and health needs of patients and service users on a continual basis in partnership with colleagues, families, and carers, referring to others for reassessment, when required The Nursing Associate will actively contribute to an effective learning
environment and support others in their learning. Nursing Associates will recognise and adhere to the Nursing and Midwifery Code of Conduct for Nurses, Midwives and Nursing Associates and work within their scope of practice following the professional standards of practice and behaviours for nurses, midwives and nursing associates and the standards of proficiency. Demonstrate ability to deliver safe and evidenced based care, contribute to ongoing assessment recognising when it is necessary to refer and escalate to others for reassessment. Provide compassionate, safe, and effective care and support to people in a range of care settings. Monitor the condition and health needs of people within their care on a continual basis in partnership with people, families, and carers. Record all care delivered appropriately and contemporaneously. Documentation must be clear, concise, and legible. Any documentation must also be signed, stamped, and dated. Communicate effectively with colleagues,
providing clear verbal, digital or written information and instructions when sharing information, delegating, or handing over responsibility for care Recognise and report any situations, behaviours or errors that could result in poor care outcomes Identify and report to a Registered Nurse clinical risk issues that impact on the patients /clients health or social care needs.
