Due to a redesign in community services, we now have a wide range of fantastic nursing opportunities including B5, B6 & B7, available across our Bollington, Disley and Poynton care community to support a building of a new team in this exciting forward thinking care community.
We are looking for Community Caseload Manager who is enthusiastic with a positive approach to join a multidisciplinary team with a vision to deliver the right care, at the right time to help improve the lives of our local residents.
Our team is based at Priorslegh Medical Centre in Poynton and covers the Bollington, Disley and Poynton area with a population of over 34,000.
Being part of our friendly and supportive team is an opportunity to take forward the expanding role of community nursing in order to meet the long-term condition agenda and the needs of our elderly population. Our dynamic management team are supportive and open to ideas and innovation., To provide holistic, high quality, safe and skilled health and social care / support to patients, carers and their families.
To work closely with member of the integrated care team to provide rapid and planned expert professional assessment and treatment interventions for patients in their own homes or other community settings, putting patients at the centre of their care and enabling patients to maximise their independence, health and wellbeing and remain in their own homes for as long as possible.
To be responsible /manage individual and team caseload of patients and carry continuing responsibility for the assessment of care needs, the development, implementation, and evaluation of care programmes.

The candidate must have excellent communication skills and hold a Specialist Nursing Practice (District Nursing) degree.
The candidate must have completed their V100 or V150 and be willing to undertake V300 prescribing.
