Full Job Description

When indicated to undertake functional assessment of behaviours that challenge for clients eligible to the service under the supervision of the services behavioural support practitioner and/or clinical psychologists.
To work alongside clients, families, paid carers and other professionals to support understanding and responses to behaviours that are challenging in line with PBS principles, Positive & Proactive Care: reducing the need for restrictive interventions (2014) and NICE Guidance (including NG11 & NG93).
Interventions will include working across a variety of settings, including family homes, residential/supported living and day services and will involve working as part of a wider multi-disciplinary team.
To assist in delivering relevant interventions to increase the knowledge and skills of the appropriate stakeholders.
To work closely with the multi-disciplinary team and wider stakeholders to deliver on the Transforming Care Agenda and local delivery plan for clients supported by the service.

+ Ensure a high standard of clinical care for the post holder's clients, providing a comprehensive and appropriate PBS service to clients, families and paid carers.
+ Play a key role in the assessment of clients who present with behaviours that challenge, undertaking both initial assessments and more detailed functional assessments.
+ To provide a range of behavioural intervention strategies based on the functional assessment of a client's behaviours which are suitable for service users living with their families and/or support staff, this will include the development of Positive Behaviour Support Plans.
+ Hold a clinical caseload, working under supervision, in collaboration with other agencies and as well as health and social care professionals.
+ To undertake risk assessments and develop risk management plans either independently or as part of a multi-disciplinary team.
+ To work with other members of the team on developing the knowledge and skill base around Positive Behaviour Support in the local borough - this may be for family carers, support staff, and/or other professionals. This may include working alongside staff that provide direct care to clients to enable them to develop their skills to work effectively with individuals with behaviours that challenge.
+ Communicate effectively with health and social care professionals and outside agencies regarding client care and progress.
+ Communicate with other members of the service regarding PBS and risk management plans.
+ Work autonomously in a variety of work contexts, including work in the community which may at times involve working with clients with complex needs in their own home and/or day service. This includes face-to-face sessions with individual clients, families and support workers. This will include working with clients across the adult lifespan and with a wide range of clinical complexity.
+ Spend sustained amounts of time with service users who may display behaviours that can be challenging, such as aggressive and hostile behaviour, who may have difficulties with communication and daily living skills and additional physical and/or mental health needs.
+ Adapt verbal and non-verbal communication to facilitate clients' understanding.
+ Liaise appropriately with all agencies involved and keep them informed of relevant information and recommended guidelines necessary to provide high standards of care for clients, within the recommended limits of confidentiality.
+ Provide a clinical service to clients from a range of minority ethnic populations and provide an appropriate specialist person-centred programme of care, taking into account language, religion, cultural beliefs and customs and adapt assessment, formulation and intervention as appropriate.
+ Plan and prioritise own clinical workload and to work with multi-disciplinary colleagues around complex cases, providing this in a setting which is most appropriate to the client and/or service needs.
+ Responsible for recording, monitoring and reporting on clinical work and communicating clinical information to a variety of recipients (e.g. service users, families, carers, other professionals, and statutory and voluntary organisations) orally, in writing and electronically, and as specified by the service. In addition, to maintain client records as an autonomous practitioner.
+ Responsible for developing and maintaining knowledge of local resources and developing working relationships with relevant statutory, voluntary and community groups and organisations.
+ Ensuring client's care needs are reviewed regularly and care plans are adjusted to meet individual's needs.
+ Other relevant duties as discussed with the Lead Clinical Psychologist.
