Full Job Description

This is an exciting opportunity to work within our dynamic and innovative LARCH service The LARCH Team are an in-reach care sector support service that respond to the changing needs of the care sector population. To improve health outcomes for people to maintain optimal health within the care sector. Reducing unnecessary emergency admissions and to maintain good quality of care in collaboration with community services and neighbourhood working. The team will co-ordinate their work across the county to reduce the number of avoidable admissions to hospital from care homes and to improve the experience of care home residents when they become unwell by working collaboratively with Care homes so that residents receive the right care, at the right time, in the right place, by the right people, The Practitioner will support the development, implementation and evaluation of programmes and pathways of care and work autonomously to manage their workload, working across the county using a multi-disciplinary and multi-agency approach to embed high standards of care within the care homes in Somerset. Mentoring role for care home managers and LARCH colleagues Health assessment reviews in home closures Clinical skills training / competency assessment Provide support and education to the PCN representatives and care providers supporting the Health and Wellbeing elements of the Enhanced Health in Care Homes framework. Treatment escalation Plan (TEP)education, quality auditing and complex TEP support Form part of the multidisciplinary team that responds to data and trends identified in the Quality Assurance Framework meetings for all care sectors, and other intelligence sharing platforms, to proactively address concerns and minimise potential escalation and impact to the wider system. Lead and support system innovation in relation to the prevention of hospital admissions and improving outcomes for those living in care homes, such as new models of care, championing roles of urgent crisis response, and Hospital@Home services. Promote and link with priorities around discharge, frailty and system flow.
