Full Job Description

TO SUPPORT CHILDREN/ADULTS/ELDERLY WITH DEMENTIA AND ALL FORMS OF DISABILITIES BY PROVIDING THEM PERSONAL CARE IN THEIR HOMES ACCORDANCE WITH DISCOVERY HOMECARE'S AIMS AND OBJECTIVES.

Main Tasks

1) To provide children/adults/elderly with mental illness, dementia and physical disabilities with personal care in their household.

2) To work in partnership with service users and carers in a professional relationship to provide a safe personal care in their home to meet their individual daily needs.

3) To support and consult with families/carers regarding the carrying out of agreed care plans for service users.

4) To understand and comply with the Qalb's Equal Opportunities Policy.

5) To offer independence in-house training to all staff to enable them to effectively perform their care role to support children, adults and the elderly in their homes.

6) To liaise with family/carers, Social Workers, and other professionals relevant bodies with a duty of care to service users as required.

7) To be familiar with the Policies and Procedures of The Qalb Centre, and also with The London Borough of Tower Hamlets Policy and Procedures guidelines.

8) To work flexible hours.

9) To provide daily written records and verbal reports regarding work with service users.

10) To participate in staff meetings and regular individual supervision sessions with the Line Managers/Coordinators.

11) To escort users whenever and wherever appropriate in their communities.

12) To undertake appropriate training recommended by the Homecare.

13) To carry out the above duties and tasks in the homes of service users.

14) To carry out other duties as may, from time to time, be required.

1) To have knowledge of The Children Act 1989 and the amended Children Act 2004, Mental Health Act 1983 amended by the Mental Health Act 2007

2) To have knowledge and experience of the needs of disabled children/Adults / Elderly with dementia and other forms of physical disabilities.

3) Ability to demonstrate good Child Care/Adults Care Practice.

4) Experience of working with children/adults with disabilities and elderly with dementia including children with Autistic Spectrum Disorder in a variety of settings, in the local community and the family home.

Abilities/Aptitudes

1) Ability to work as part of a team.

2) Ability to understand the needs of children/adults with disabilities, adults with mental health and the elderly with dementia.

3) To be aware of the community languages including symbol, Makaton and British Sign Language.

4) Understanding of the different language needs.

Other Requirements

Education and Training

1) Professional qualification NVQ Level 1 and 2 relevant to children/adults care

2) Ability to communicate effectively with other agencies, service users and carers.

3) Ability to write simple and complex reports.
