Full Job Description

We are working closely alongside a Local Authority in Lewisham to assist with the appointment of a Case Management Officer, on a 3-month contract, highly likely to be extended at clients discretion. Please apply with your CV for immediate consideration., This role involves responding to Adult Social Care enquiries, engaging with commissioned unpaid carer service providers, and ensuring accurate record-keeping within the Adult Social Care system. The postholder will also undertake Carers Assessments when required, provide specialist interventions, and retain a small but complex caseload of younger and older adults requiring frequent support. Responsibilities:

  • Carer Support & Assessment: Respond to Adult Social Care enquiries regarding unpaid carer services. Support unpaid carers through assessment, guidance, and community resource connections. Complete Carers Assessments when required.
  • Case Management & Intervention: Manage a small but complex caseload, providing specialist intervention and regular support. Ensure unpaid carers receive appropriate help and resources.
  • Information & Advocacy: Support carers by providing information on rights, services, and entitlements. Act as a liaison between carers, social care teams, and external service providers.
  • Safeguarding & Risk Management: Ensure risk management and adult safeguarding protocols are effectively embedded in all interactions and appropriately responded to.
  • Outreach & Community Engagement: Assist with outreach initiatives, community engagement programs, and service development activities to promote carer support. Participate in customer surgeries to enhance service accessibility.

    No specific degree required, but a background in Social Care, Health, or Community Services is desirable., Experience working in adult social care or carer support services.
  • Experience managing customer enquiries and casework.
  • Strong understanding of social care service delivery and safeguarding practices.
  • Ability to work in pressurised, frontline service environments.
