Full Job Description

Be the lead nurse assessor for children and young peoples continuing care (CC) with a responsibility to lead and undertake effective CC assessments and reviews of children and young people lead and undertake case management to children and young people eligible for continuing care Liaise with clients, families, advocates and providers in relation to children and young people eligible for continuing care Undertake innovative work to source particular and bespoke services (including assistive technology and equipment) for children and young peoples CC to meet more challenging and non-typical needs. Support the delivery of the ICBs statutory duties in relation to the National Framework for Children and Young Peoples Continuing Care Play a part within the AACC clinical improvement programmes around quality, performance and service delivery for children and young peoples continuing care Coordinating work across different organisations and stakeholders to develop innovative and bespoke
solutions for children and young people eligible for continuing care Leading hospital avoidance and discharge planning activities as this relates to children and young people eligible for continuing car Sourcing emergency packages for children and young people eligible for continuing care, as required. Leading communications with children and young people eligible for continuing care, their families, carers and advocates to ensure that they are at the heart of CC decision-making at every stage. Providing ongoing oversight of service delivery for children and young people eligible for continuing care Supporting activities related to the multi-agency team around the child (Match) forum. Leading on relevant activities in relation to CC and Special Educational Needs and Disabilities (SEND) such as contributions to Education, Health and Care (EHC) Needs assessments as required. Leading relevant AACC elements of the local area SEND improvement agenda as required. The provision of expert advice
around children and young peoples continuing care in support of key AACC commissioning and market development projects as required. Lead communications with eligible clients, their families and carers on behalf of the AACC in relation to children and young peoples continuing care Support work that helps ensure that communications and relationship-management strategies of the AACC are effective, collaborative, outcomes-focused and place the AACC at the heart of decision-making and partnership working in the ICB Support and lead sometimes fraught and challenging communications with partners around AACC services, including funding decisions as required. Support and lead communications with key internal AACC partners as required including children and young peoples safeguarding, quality, commissioning, contracting, the Designated Clinical Lead for Special Educational Needs and Disabilities (SEND) and the Clinical Lead for Children and Young People.
