Full Job Description

To undertake comprehensive assessments of need for children with complex health needs, utilising specialist health knowledge and experience. Always maintain a person centred and multi-agency approach to needs assessment and care package development. To work in partnership with all relevant stakeholders in the provision of continuing health care packages to enable the child or young person to attain their optimum life experience and outcomes. To facilitate difficult and challenging conversations and communicate highly sensitive and complex information using high level communication skills to a range of audiences including medical/non-medical professionals, parents/carers, children/young people. To use advanced analytical skills in continuing health assessments for children in agreeing levels of care, funding and developing bespoke packages of care within agreed eligibility and criteria. To support parents/carers and young people to hold a personal health budget where eligible and desired.
To review at least annually packages of care, outcomes, and personal health budgets, ensuring they continue to work effectively for the child and their family. To meet statutory duties under the SEND Code of Practice, engaging in the Education, Health, and Care planning process to ensure alignment of EHCP and Continuing Care. To attend SEND decision makings panels as a health representative or to present continuing health care assessments for peer review. To support professionals across education, health, and care services by offering timely, effective specialist health support and information.
