Full Job Description

An experienced community clinical practitioner with theoretical and practical knowledge in palliative care. The purpose of this role is to provide specialist nursing care, advice, guidance and leadership.

  • Assess, plan, implement and evaluate evidenced-based nursing care.
  • Provide specialist advice and excellent clinical practice.
  • Work autonomously and manage own workload/caseload.
  • Provide specialist education and training to staff.
  • Work collaboratively and co-operatively with clinical colleagues.
  • Demonstrate the Health Board's values and ensure that others demonstrate the same values.

  • This is a rewarding opportunity to become part of the community specialist palliative care service in Carmarthenshire. This is a chance to join a team that are committed to improving quality of life for people, and those closest to them, who require symptom control, psychological care and are living with a progressive, life limiting illness. We are looking for an experienced, dynamic and motivated nurse to join our Palliative Care Team. Previous experience of working in community teams/settings is desirable, however all applications will be considered., The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

    Hywel Dda University Health Board is the planner and provider of NHS healthcare services for people in Carmarthenshire, Ceredigion, Pembrokeshire and its bordering counties. Two integrated care centres (Aberaeron and Cardigan, Ceredigion).Community facilities, including: 48 General Practices (GP surgeries), 49 Dental Practices, 98 Community Pharmacies (chemists), 44 General Ophthalmic Practices (including eye health and low vision services), 38 sites providing mental health and learning disability services, care within your own homes. Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
