Full Job Description
The post holder will provide a comprehensive specialist support service to patients/families with a suspected or proven haematological disorder.
The aim of the post is to provide expert care, advice and support to patients with differing haematological conditions, both malignant and non-malignant.
The post holder will liaise with other healthcare professionals both in secondary and primary care settings, ensuring coordination and continuity of care from the point of investigation and diagnosis to end of life care.

1. To attend weekly MDT meetings and to take an active role in contributing to patient discussions; to input relevant data into Somerset, and to organise the agreed patient outcomes following each meeting
2. To assess the needs of both patient and family, ensuring provision of client centred care which focuses on the promotion of both health as well as illness
3. Refer on to Allied Health professionals and other relevant Health Care Professionals within the Trust and in the Primary care setting as appropriate.
4. To provide patient and carer advice, whether that be face to face or by telephone.
5. To triage calls in order of severity/importance, identify concerns and priorities and to put in place appropriate support
6. To interpret and monitor blood results and to put in place appropriate action according to the management plans, bringing abnormal results to the attention of the Consultant to minimise patient risk and ensure timely actions.
7. To book blood and platelet transfusions according to individual blood results & specific medical /CNS management plans.
8. To offer psychological support and empathy to patients and carers where appropriate and to refer on for specialist input as needed
9. To provide symptom control advice and support to patients/carers
10. To attend specific Haematology medical clinics to support the Consultant Haematologist and be available to support patients with a known Haematological diagnosis
11. With the support of the Consultant Haematologist review ward patients and input into their individual care plans
12. To provide support and training to ward based medical/nursing teams as appropriate
13. With the support of the Consultant Haematologist to review/assess Day case attenders on the Macmillan Unit or PIU as required
14. To participate in the Holistic Needs Assessment of patients with a Haematological malignancy and document an individual care plan
15. To support both patient and carer in End of Life care planning discussions and implementation of care plans within the secondary or primary care setting
16. To establish and maintain close working collaboration with consultant and other key colleagues, to facilitate the best possible patients care
17. To plan and coordinate patients follow up treatment/outpatient appointments to ensure correct and timely delivery of care
18. Ensure completion of accurate, timely documentation in accordance with hospital policy and standards.
