The salary scale above has been agreed as part of the NHS Agenda for Change pay award for 2024/2025 and will be implemented in November 2024 backdated to 1st April 2024 where applicable., The post holder will work in partnership with the Governance Team to develop an integrated governance framework for Childrens services, in line with the wider Health Board strategy.
The post holder will promote a culture of transparency, honesty and accountability within Childrens Services.
In conjunction with the Governance Team, will develop clinical risk management systems, promoting proactive risk management and Governance within the Integrated Health Community, + Develop Clinical Risk Management initiatives and strategies with support of Governance Team
+ Coordinate and facilitate risk assessment within the department.
+ Plan and organise monthly Clinical Risk meetings.
+ Organise and facilitate weekly Datix meetings.
+ Grade all clinical incidents, and ensure they are logged into the department system for collating clinical incident statistics
+ Following trend analysis, through recommendation to the Clinical Network manager, contribute to the Improvement Agenda
+ Ensure timely and effective investigation of incidents by the appropriate members of staff.
+ Co-ordinate all Serious Incident Reviews and participating in the investigation of such incidents.
+ Manage highly complex, sensitive or contentious information, to include analysis of data and reporting on the same.
+ Oversee the perinatal morbidity review tool process.
+ Report serious incidents to the Legal Department and Welsh Government (where applicable).
+ Be responsible in conjunction with the service managers for ensuring actions from clinical incidents investigations have been actioned within the Integrated Health Community and ensure an integrated approach is in place to learn lessons from incidents and litigation.
+ Ensure all aspects of Health and Safety are adhered to, reporting and untoward incidents appropriately.

+ Provide statistics as required by the Integrated Health Community.
+ Ensure the practice of self and others is compliant with local and national policy.
+ Work in highly emotional circumstances, dealing directly with families and staff who have experienced bereavement
+ In conjunction with the Governance Team, be responsible for ensuring that training and education in relation to all governance issues is provided within the Integrated Health Community (e.g. Root Cause Analysis; Incident Management and Record Keeping).
+ In collaboration with other members of the multi-disciplinary team participate fully in multi-disciplinary audit meetings.
+ Ensure that the Integrated Health Community has a robust mechanism for acting on the recommendations of all national confidential enquiries (e.g. MBRRACE, NNAP, NMPA) and all NICE guidance
+ Take a role in the evaluation of clinical policies, protocols, guidelines and clinical pathways.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
