Community Mental Health Assertive Outreach Support Worker

Regular clinical supervision and training opportunities will be provided to the successful candidate. The post holder will be a key member of the CMHT, working directly with service users under the care of the CMHT, as part of the Multi Disciplinary Team which provides care, treatment and support to people with a functional mental illness. The post holder will provide support to those service users in line with the clinical pathways within the CMHT - namely Assertive Outreach and Dual Diagnosis pathway, as outlined within the CMHT operational procedure. The role also involves short term periods of care to assist service users to develop independent living skills. The post holder will provide support, advice and practical assistance to an allocated group of service users under the direction and supervision of a qualified member of the staff team, following a care plan agreed with the service user to facilitate goal planning, achievement and recovery. The post holder will support carers of service users and will provide a carers clinic where they will undertake carers assessments.

Whilst this post requires no professional registration, experience within mental health is essential. The post holder's professional development will be supported by the service through regular supervision and training opportunities.
