We have an opportunity for a forward thinking, highly motivated Registered Mental Health nurse to join us working as Community Mental Health Nurse.
The job will build on your experience as a developing or experienced staff nurse to afford an excellent platform for further career development.
There will be opportunities for further training and regular supervision will be provided. We are keen to develop nurses with advanced skills (or desire to learn). Your development needs will be supported. You will deliver the highest quality holistic health care, incorporating the mental and physical health needs of our patients. You will ensure measurable improvements in wellbeing.
You will be expected to take personal responsibility and accountability for your patients needs, coordinating services and care needs as required throughout the care pathway.
This job is challenging and intense, but is one of the most rewarding jobs within the mental health care setting. Please do not hesitate to make contact for more information if you wish to discuss the post prior to applying., To deliver evidence based holistic care to clients within the context of a multidisciplinary, multi-agency Community Mental Health Service.
To take responsibility for a clinical caseload providing comprehensive assessment, care and treatment to clients referred to the Community Mental Health Team in accordance with Parts 2 and 3 of the Mental Health (Wales) Measure 2010.
Foster robust working relationships with all partners and key stakeholders, in particular Local Authority, Primary Care and the Third sector.
Contribute to the continuing improvement and development of community mental health services.
