Full Job Description
A key aspect of the Community Psychiatric Nurse role will be advising and supporting care home staff to improve their skills and confidence to manage behaviours and promote person centred care planning. Specialist knowledge of the nursing issues associated with dementia in a residential setting and a high level of communication skills are crucial. The County Dementia Outreach service covers the County of Nottinghamshire and operates within a "virtual team" service model with clinicians being based within locality based community mental health teams., A full UK driving licence and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
Experienced, NMC registered Band 6 Community Psychiatric Nurse with substantial experience of working with people with dementia in an inpatient/ residential setting and or community setting
Advising & support care home staff to improve their skills and confidence to manage behaviours & promote person centred care planning
Specialist knowledge of the nursing issues associated with dementia in a residential setting, high level of communication skills are crucial as are good written, verbal and IT skills
Robust understanding of risk assessment and management, experience of managing a case load essential
Demonstrate understanding of the mental and physical health and social needs of the client group, demonstrate a working understanding of Safeguarding issues and DOLs.
Time management and organisational skills and ability to work under pressure, nursing student mentor.

The County Dementia Outreach Service operates within a "virtual team" model with clinicians being based within community mental health teams. We utilise dementia friendly, non-pharmacological, person centred care approaches working collaboratively with our residents, their loved ones and care home staff to enhance wellbeing. Our remit is to reduce distress and behaviours of concern. We aim to reduce inappropriate admissions to hospital and maintain appropriate care placements.
