Within the role you will be expected to work with more complex patients providing holistic evidence-based care, assessing, planning, implementing the treatment requirements of the practice population. To work collaboratively and maintain key working relationships with managers, service heads and leads. Establish and uphold proactive connections with stakeholders, including ICBs, NHS Organisations, CHC, PCNs, Plymouth City Council, as well as private/voluntary sector (i.e. St. Lukes). JOB PURPOSE This role will help to ensure that all people with moderate and severe frailty have an agreed Treatment Escalation Plan in place that can be called upon during times of escalated need. The post holder will visit identified people with frailty in the community, and work with them in a sensitive and empathetic fashion to agree a robust TEP that meets the needs of the individual. The role will link closely with the Frailty Virtual Ward team, which will comprise of a range of professionals, including Medics, Advanced Care Practitioners and a range of other healthcare professionals. In addition to these duties, the post holder will be expected to provide backfill into nursing teams, within their existing level of competency. This will allow current staff to train to undertake TEPs, increasing the overall number of people in the workforce who can have these conversations with people. It is the aim that 100% of people who have been identified has having moderate or severe frailty
