Theservice is responsible for delivering a wide range nursing care to houseboundpatients, which includes wound management, chronic disease management,promotion of self-care, terminal / palliative care & the administration of IVanti-biotics for admission avoidance. Successfulcandidates will maintain highquality effective nursing care in the community as part of the wider integratedcare team. Theteam is committed to personal development, training & wellbeing. At the time of advertising, this role does meet theminimum requirements set by UK Visas and Immigration to sponsor candidates towork in the UK., Asa Neighbourhood Staff Nurse you will support the District Nurse in carrying outnursing care for patients primarily within their home but you will alsoparticipate in clinic sessions. Enthusiasmand the ability to use your own initiative are essential, along with goodwritten and verbal communication skills and the ability to manage your timeeffectively. Youwill need the ability to work both as part of a team and as a lone worker,along with the use of a vehicle to undertake any necessary travel in connectionwith the duties of the post. Youwill be allocated to a specific Neighbourhood; however, it should be noted thatyou are employed to work across the service and may be required to move basesif / when required. Youwill be required to work flexibly as the service operates 8am 8pm. Fulland Part time positions are available. If you are using AI to enhance yourapplication, please disclose this in your NHS Jobs application form., Undertake duties under the supervision of the District Nurse Deliver evidence basedplanned care, which is safe, effective, in a manner which is caring,compassionate and enables individuals to reach an optimum level of functioning,independence, rehabilitation and recovery. Adapt and provide a wide rangeof nursing care in home and community based settings Be accountable for assessing, planning, implementing andevaluating care for patients, reporting and escalating relevant information toappropriate health and social care professionals. Workin an integrated andpartnership way with primary, secondary, social care, the independent andvoluntary sector and others to improve the health and care of individuals,families and communities, particularly the most vulnerable. Undertake and maintain the required statutory, mandatory andcore training and preparation to meet the specific clinical needs of theservice. Participate with the training of learners and participate in staff inductions where appropriate. Delegate work to other staff members within the team. Establish good working relationships with the Primary Health Care Teams

  • Ability to undertake the duties and demands of the post. A satisfactory sickness record over the previous 2 years (subject to the need to act with fairness and equality of opportunity, particularly where the sickness is related to a disability and / or pregnancy).
