An exciting opportunity has arisen for a registered nurse to join our forward thinking and award winning Care Community Team in Winsford, to cover a 7 day service. There are full (37.5 hours per week) and part time (22.5/30 hours per week) posts available.
This post requires effective communication and interpersonal skills and the ability to be adaptable, reliable and approachable. The successful applicant will have a sound knowledge of recent changes within the NHS and knowledge of the political drivers to provide care closer to home.
As part of a team you will be responsible for the assessment of nursing care needs, implementation and evaluation of care programmes and the delivery of high quality holistic care within the community setting, and within our wound care clinics. You will be expected to support your peers, the team leader and the organisation in promoting the Trusts values and behaviours., We are looking for Band 5 staff nurses to support the patients in the Winsford Care Community Team. The integrated team consists of Complex Case Practitioners, District Nurse/ Case Managers, Macmillan, and Therapy teams delivering high quality, efficient and safe care.
We are looking for enthusiastic caring and compassionate people who possesses a good clinical skill set. You will also be able to holistically asses patients needs whilst ensuring health promotion and quality improvement to meet the needs of our patients. You must have a good knowledge of modern IT software applications.

You will need to be able to work independently, possess good time management and organisational skills and be expected to be innovative and flexible to work in other areas if required to meet service need.
Mid Cheshire Hospitals NHS Foundation Trust (MCHFT) provides good quality, safe and effective healthcare to the people of Cheshire and beyond. The Trust, which manages Leighton Hospital in Crewe, Victoria Infirmary in Northwich, and Elmhurst Intermediate Care Centre in Winsford, was established as an NHS Trust in April 1991 and became a Foundation Trust in April 2008.
