An exciting new opportunity has arisen for an enthusiastic, motivated & experienced Community Nurse to join our Integrated Community Team (ICT) within the referral centre.
This is a new role to support the current demands of the community nursing service which will guide and inform future service development.
The referral centre promotes a dynamic and innovative way of working, in the triaging and response to patient referrals. You will complement our current team of professionals, assisting with the day to day running of the referral centre and supporting patients by responding to unscheduled calls following appropriate clinical triage and geographical planning. This will involve shift working within the core hours of the community nursing service between 8am-8pm and will also require occasional weekend support. As the ideal candidate for this post you would be an experienced and highly competent registered nurse looking for a new challenge and seeking an opportunity to make a real difference, ensuring you are professionally accountable and responsible for patients' care., The post holder will be based in the referral centre at Independent Living Centre and primarily work across Cheltenham locality. However, being part of our wider ICT, this may also provide the opportunity to work across different nursing teams. The role involves visiting patients in their own homes, who require assessment and intervention with a view to prevent any deterioration or unnecessary hospital admission. You will also be working collaboratively with the wider multidisciplinary team to support the management of complex health and social care needs.
