Full Job Description

As a Bed Navigator, you will play a crucial role in providing flexible, person-centred support to individuals in various locations, helping them to maintain independence, access essential services, and achieve positive outcomes.
Support Clients: Work with individuals facing challenges such as housing instability, financial management, and accessing healthcare
Collaborate with Services: Liaise with housing providers, social services, and other organisations to provide comprehensive support
Adapt to Needs: Travel between locations and provide tailored support based on each client's situation
Empower Independence: Equip clients with the skills and resources needed for sustainable independence
Core Responsibilities:
1. Client Engagement:
o Conduct needs assessments and create personalised support and safety plans with a trauma-informed and strength-based approach.
o Advocate for clients, helping them access housing, welfare benefits, healthcare, and employment opportunities
2. Case Management:
o Maintain accurate and timely records on all client interactions.
o Regularly review support plans to track progress and adjust according to changing needs
3. Crisis Management:
o Identify and respond to crises, such as eviction threats or health emergencies
o Collaborate with external agencies to provide immediate support when needed
4. Professional Collaboration:
o Build strong relationships with local service providers and community organisations
o Represent the rganization in multi-agency meetings
5. Person-Centred Approach:
o Respect clients' autnomy and choices and build trust through active listening and empathy.
6. Cultural Sensitivity:
o Be aware and respect clients' diverse backgrounds, beliefs, and experiences.
7. Confidentiality:
o Maintain confidentiality and protect client information in line with organisational and legal requirements.
8. Self-Care:
o Maintain personal boundaries and seek support when facing challenges.

Relevant roles will be subject to a successful disclosure at an appropriate level from Disclosure and Barring service (DBS)

+ Experience working with vulnerable individuals, particularly those experiencing homelessness, addiction, rough sleeping, poor mental health, or offending
+ A non-judgmental attitude and an ability to build rapport with individuals from diverse backgrounds.
+ Strong communication skills and the ability to work collaboratively with multiple agencies.
+ A proactive and adaptable approach, with the ability to navigate complex situations.
+ Ability to work independently and manage time effectively.
+ Knowledge of housing and welfare systems is desirable.
