Full Job Description
We are looking for a committed and resilient individual to join our unique team as we continue our Staying Close Journey.
You will be part of a multi-disciplinary team including Personal Advisors, Education, Employment and Training Worker, Housing Officer and Emotional Health Practitioner and line managed by the Staying Close Team Manager.
Our model is based on relational practice, and we will support young people who are living in their children's residential homes to receive additional support as they transition into adulthood.
You will be responsible for a caseload of young people that require an intensive, strength-based wrap around support across key areas of their transition into independent living. This will include supporting them with their emotional health needs, offering 1:1 support and group work.
You will need to be able to support young people to access community services in relation to their emotional health and provide appropriate challenge to ensure they are offered the right support.
Some of our young people live out of area and therefore travel will be required to ensure our most vulnerable young people are still able to receive the intensive support we hope to offer. You will receive essential car user allowance and be subsidence for mileage in line with the council policy.

We are looking for someone with a positive outlook who is passionate about supporting young people as they prepare for adulthood and independence.
You will need to have a good understanding of the issues our vulnerable cohort can face and support crisis intervention.
You will need to be resilient, solution focused and work strongly as part of a team.
Staying Close is a new and relational based offer, we are therefore looking for an innovative, energetic and creative individual who has experience of working with vulnerable young people and is keen to support the development of the service.
We are a small, friendly team that sits alongside The Leaving Care Team and work closely with our wider partners. You will need to be confident in closely co-working and supporting the team in the best interest of the young people we support.

Grade: I scp 31 £40,476 to scp 35 £44,711 plus Essential Car User Allowance up to £963 pa
Contract: Fixed Term to March 2026
Essential Requirement: Full UK Driving Licence
Essential Qualification: Social work, Nursing or other relevant professional qualifications i.e., Psychology degree/specific therapy qualification such as CBT Diploma
