You'll be a key part in making a big difference each day as you help our customers achieve their hopes and aspirations. How amazing is that! We have fabulous opportunity for you to join our awesome team. You have a genuine passion to support our services in delivering aspirational person-centred housing related support. You will be working as part of a team with two other Health Coaches alongside Support Coordinators, Senior Client Services Managers, and a Mental Health Practice Lead, delivering person-centred mental health recovery support and encouraging independence. Our customers live in shared houses of up to 5 people across Exeter and Torbay. Typical day as a Health Coach

  • You'll transform lives by coaching customers as part of a multi-disciplinary team alongside our support and care colleagues as well as with external partner organisations.
  • Coaching our customers who are experiencing mental health issues to take control and ownership of their health.
  • You'll carry out assessments and create individual health plans to enable our customers to adopt healthy behaviours. Focusing on what is strong, not what's wrong.
  • Deliver interventions to support customers to achieve their health goals identified in their plan.
  • You'll be a change maker there to motivate others to kick their unwanted habits and activate personalised healthy lifestyles!

  • You will be expected to work Monday to Friday 09.00-17.00, working in our Exeter Service.
  • Although you will be lone working the customers you will have daily contact and communication with the rest of the team and we do get together for team meetings, training and often, just a cuppa.
  • You're able to use technology. We have a variety of systems we use for reporting, creating support plans, emails, Teams calls and so on
  • You'll be given the latest IT Kit, which could include a Surface Pro Tablet and Smart Phone, making it easier to complete support plans on the go.
  • You'll need an Enhanced DBS check done and we pay for that.

    Experience working in the community supporting people, ideally with their health
  • Experience coaching others
  • Your knowledge of the inequalities and barriers that impact on health and wellbeing
  • You'll need to bring your people powers in motivating and supporting others, with your ability to deal with conflict and challenging behaviour
  • If you have an NVQ in Health and Social Care, Health Trainer and/or Coaching qualification then that will be the cherry on top!
  • You'll need to travel around to spend time with your customers, you can do this role using public transport or if you drive, to get from A to B, you'll need a vehicle insured for business purposes. The great news is that we'll pay your mileage between site visits!
