Full Job Description

The role of the health navigator will be to work as part of a multi disciplinary team to support vulnerable people through the criminal justice systems. The navigator role is an essential role working within police custody in the Worthing area and helps with assessment and support for both adults and young people. The health and justice navigator will work alongside qualified staff in undertaking assessments and liaising with the police and partner agencies.The aim of the health and justice service is to support vulnerable people who enter into the criminal justice system with the aim of supporting any health needs. The role can be challenging and very rewarding and it is important that the people who work in such a setting are able to build rapport and undertake assessments of health needs. A good understanding of mental health will be important and also other health related issues that people may experience., o To carry out health assessments.o Screening for neurodevelopment conditions.o Liaising with the police.o Liaising with partner agencies.o Developing support plans and care plans.o Supporting vulnerable adults and young people.o Working as part of a multi disciplinary team.o Supporting qualified staff.o Participating in team development and team approach.
