Full Job Description

As a Health Care Assistant you will assist the Specialist Nursing Team nursing team to deliver an efficient service for patients post renal transplant.

You will record patient information and provide clinical support as required to help ensure the efficient and effective running of the service.

You will provide excellent customer service to all internal and external customers.,

  • Establish and maintain communication with patients, relatives and carers and the multidisciplinary team to be able to provide holistic patient care.o Develop own skills and knowledge and provide information to others to help their development.o Monitor and maintain health, safety and security of self and otherso Assist in maintaining and developing services.o Ensure own actions help to maintain quality.o Assist in assessing people's health and wellbeing and related needs.o Assist in planning, delivering and evaluating programmes to meet people's health and wellbeing needs.o Assist in the use of technology for measurement, monitoring and treatment.o Ability to work flexibly, In return, we can offer you a job role with purpose and flexibility.
