Full Job Description

This is an exciting time to be joining our team and you will have the opportunity to be involved in developing heart failure care in Brighton as the service develops further. We will provide training and support to the successful candidate., Part of the heart failure specialist nursing team providing expert care to chronic heart failure patients, based at the RSCH, Brighton site but may need to provide cover across site on occasion.

Provides autonomous specialist care to inpatients, outpatients, ambulatory care and virtual ward patients as per local service development.

Provides evidence based heart failure care in order to optimise therapy and outcomes for patients, and to reduce unnecessary hospital admission.

Shares responsibility for the ongoing development of the heart failure service, including developing pathways for admissions avoidance.

Provides specialist education and training to hospital staff and community staff (GP's / practice nurses).

Provides specialist advice to patients and / or their carers to influence better care and outcomes for patients

Undertakes research and clinical audits

You will need to be a degree level nurse who has had significant experience of caring for heart failure patients, with the ability to work both independently and as part of a team. You will need to demonstrate leadership skills and experience of clinical audit.
