Full Job Description

We are looking for a motivated and dynamic pharmacist to join the haemoglobinopathy service, and contribute to the development and implementation of joined-up hospital and community protocols for drug therapies. The remit will include patients living with Sickle Cell Disease, Thalassaemia and rare anaemias within North East London. You will work with an expert multidisciplinary team, to improve patient awareness and adherence to medication as well as enhanced access to medication. The remit will include drug therapies such as hydroxycarbamide, iron chelation and analgesic medications-including responsible management of opioid therapy. There will also be an opportunity to develop pathways and protocols to enhance access to new therapies. The post will help to provide a comprehensive clinical pharmacy service to patients, the multidisciplinary team and provide highly specialist support to the wider pharmacy teams across NEL. You will also help to develop clinical guidelines and act as a key member of the MDT., You will assist the service in the development of effective and appropriate community services within North East London including

  • Mapping of the current pathways for provision of therapies and therapy monitoring in the hospital community settings across NEL.
  • Help shape how services supporting patients are delivered and accessed in the community.
  • Support the development of effective pathways of community prescribing and medication monitoring and compliance within community settings relating to sickle cell and thalassemia.
  • The post holder will participate in education and training of Pharmacy staff at Bart's Health and will assist in the provision and development of clinical pharmacy services within the Network.

  • Post-registration training and experience in an acute hospital NHS setting
  • Experience of having worked in organisation managing care of people with haemoglobinopathies
  • Experience of multi-disciplinary team working joint work and inter-agency collaboration.
  • Experience of working within multi-cultural populations.
  • Experience of safe dispensing and medication monitoring in different cultural contexts.
  • Demonstrate the ability to appropriately recommend, substantiate and communicate therapeutic options for patients
  • Desirable criteria
  • Experience in the management of haematological disorders and / or chronic pain.
  • Clinical experience working in Haematology services
  • Experience of providing patient consultations with regards to medications
  • Experience of supervising trainee pharmacists having completed the relevant training.
  • Experience in leading and supervising research, quality improvement or audit projects.
  • Planning and implementing significant service developments and organisational change.
  • Skills Essential criteria
  • Excellence in leadership skills; including the strategic development of services and proven success in project management of quality initiatives.
  • Excellent communication skills (written and able to complete documentation accurately) with attention to detail
  • Able to communicate complex information effectively to healthcare porfessionals, colleagues, patients and carers
  • Good ability to use computer systems including excel and word
  • Able to work effectively both alone and in a team
  • The ability to manage and develop self and others in a busy working environment.
  • Excellent interpersonal skills, including the ability to effectively collaborate with colleagues, family, carers, and service users to deliver the best possible services with available resources.
  • Ability to identify and employ mechanisms of clinical governance as appropriate to support and maintain clinical practice
  • Desirable criteria
  • Experience in writing policies or guidelines
  • The ability to plan and deliver pharmacy services across a large service in effective, efficient, and resourceful manner., Knowledge of the pharmacology of disease modifying agents and iron chelation agents
  • Knowledge of acute and chronic pain syndromes using conventional analgesics, strong opiate, opioid and non-opioid analgesics in the context of sickle cell disease
  • Knowledge of holistic non-pharmacological approaches to managing acute and chronic pain
  • Understanding of the pharmaceutical care needs of patients, including knowledge of the medication issues at the interface of primary and secondary care
  • Awareness of current national standards, guidelines and service delivery issues related to haemoglobinopathy patients
  • Knowledge of key literature rand understanding of patient safety initiatives
  • Knowledge of legislation in relation to the client group and mental health issues, child and adult protection and equalities.
  • Knowledge of legislation and guidance relating to medicines, pharmacy, rationing decisions in the NHS and commissioning pathways
  • Understanding of GPhc revalidation requirements, Knowledge and understanding of Sickle Cell Disease
  • A good understanding of national and local priorities
  • Knowledge of the law and relevant guidance relating to medicines, pharmacy, rationing decisions in the NHS, Payment by Results (PbR) and commissioning, Masters Degree in Pharmacy or equivalent
  • Registered with the GPhC
  • Post graduate pharmacy qualification or equivalent experience (minimum post graduate diploma level)
  • Undertakes continuing professional development (CPD) and maintains a CPD portfolio, Independent Prescriber or willing to undergo training
  • MSc in Clinical Pharmacy or equivalent.
  • Management qualification or equivalent experience
  • Competence and / or specialist training in models of health psychology and / or behaviour change (e.g. motivational interviewing).
  • Member of Royal Pharmaceutical Society and Member of Faculty
