Exciting opportunities within the Stockport Intermediate Care at Home and Discharge to Assess Team.
We need people who share our excitement, embrace different ways of working and are able to work flexibly across organisational boundaries. Opportunities have arisen for registered professionals - Band 6 to work flexibly within the Intermediate Care at Home and Discharge To Assess team.
The Discharge To Assess element is a multi-professional service who assess patients upon discharge from hospital in the community .We provide individualised care to meet the person's needs and facilitate transfer for onward care into the appropriate service: Health, Adult Social care or partner agencies.
The Discharge to Assess element: to support people who are clinically optimised and do not require an acute hospital bed, but may still require care services with short term, funded support to be discharged to their own home or another community setting.
Assessment for longer-term care and support needs is then undertaken in the most appropriate setting and at the right time for the person. We provide holistic multidisciplinary interventions and support in order to stabilise patients in their own home or usual residence. This comprises a multi-disciplinary integrated team.
The Rehabilitation Element incorporates a free, short-term rehabilitation service for adults to help prevent unnecessary hospital admission, enable early discharge from hospital and prevent unnecessary admission to long-term residential care. This role will require the post holder to be flexible to cover all areas within the Active Recovery and Discharge to Assess service covering 7 days 08.00-21.00.
