Full Job Description

The role will focus on dissolving traditional barriers in ways of working, co-designing new local pathways across health and social care and establishing the required contractual arrangements across the healthcare platform. This a highly strategic role that sets out to deliver the integration strategy for Primary Care and Community Services across Merton and Wandsworth. The role will lead on key aspects of primary care and community services transformation as required by the Programme Delivery Lead, furthering the ambition for integrated commissioning across both health and social care via relevant project and programme management and pathway re-design., o Local relationship management between respective stakeholders across the two Boroughs using informed persuasion to influence others. oUtilise highly developed communication skills to synthesise and present complex information in an articulate way to successfully facilitate the change management processes. o Supporting delivery of the strategic commissioning framework as well as the National Policies and programmes of work associated primary care and community services. o Work autonomously to tight and often changing timescales, developing solutions with key stakeholders under the direction of the Deputy Director of Primary Care and Community Services. o Actively build productive relationships with GPs, practices, federations and other stakeholders across primary, community and secondary care. oManagement and delivery of end-to-end project lifecycles, service and quality improvement via iterative methodology to improve patient outcomes and value for money.,
