Full Job Description

An exciting opportunity has now arisen within the Trafford Specialist Palliative Care support Team for an Assistant Practitioner.

Working within the Specialist Palliative Care Team, contributing to the provision of a co-ordinated and comprehensive service for patients receiving specialist palliative care including patients resident in care homes in Trafford.

The post holder will work as part of the Specialist multi-disciplinary team comprising of Nurses, Occupational Therapist and Physiotherapist. The team aims to provide effective palliative care and support rehabilitation to patients with a diagnosis of cancer at any stage of the patient's illness from diagnosis onwards.

The post holder will work as part of the Specialist multi-disciplinary team comprising of Nurses, Occupational Therapist and Physiotherapist. The team aims to provide effective support rehabilitation and/or palliative care to patients with a life limiting condition requiring palliative intervention.

The post holder will assist other professionals in the implementation, delivery and evaluation of rehabilitation and education programmes to promote independence and encourage improved health, functional capabilities and quality of life.

The post holder will hold a small caseload under the supervision of the responsible clinician, supporting patients with life limiting illness to maximise their health and enable them to be cared for in their preferred place.
