Medicines Optimisation Assistant

We have an exciting opportunity at Diamond Health Group to recruit a Medicines Optimisation Assistant. This is an innovative role working closely with our wider medicines management team of prescription clerks and a practice pharmacist., You will use your knowledge of pharmaceuticals to make simple changes to patients medication records as directed by hospital consultants or other care providers. You will work closely with patients via telephone to streamline repeat prescribing, minimise clinical risk and reduce waste within primary care. You will network with our patients and community pharmacy colleagues to provide a seamless service.

Post holders are required to have relevant GPhC accreditation (NVQ2 in Pharmacy Services) or equivalent.

We can offer flexibility with hours for this position and up to full time working hours will be considered. We therefore welcome applications for full or part-time working.

We offer 27 days annual leave per year (pro rata) and enrolment in the NHS Pension Scheme.
