Full Job Description

This role will be hosted by Sheffield Health and Social Care (SHSC) and work on behalf of Sheffield Place for the people of Sheffield. This post is a system post and will work alongside all system partners from the Sheffield Integrated Care Board, Sheffield Health and Social Care and Sheffield City Council. The role will lead and work within the wider Sheffield adult health and social care provision providing professional leadership and clinical expertise to address social care challenges within SHSC with the aim to eliminate all delays from hospital. SHSC want to reduce the risk of harm to the person by delays in their transfer of care and we want to improve people's outcomes, home is best. The Mental Health Discharge Lead will provide operational leadership and coordination to enable transfers of care from mental health acute inpatient hospital beds and support recovery in the community., The Mental Health Discharge Lead will have significant opportunity to shape robust systems and processes as part of the Home First Programme of work, and will be integral to the delivery and implementation across SHSC and the wider system. You will develop the data systems and process to enable a system wide understanding of the hospital discharge policy, supporting a collaborative approach to understanding and deliver care based on the needs of the population of Sheffield. This role is responsible to SHSC Head of Service who will drive the longer-term strategy for discharge including capacity and demand planning, pathway design and integration recognising the national intention to develop models to integrate alternatives to admissions and discharge planning.Working for our organisation At SHSC, we prioritise the well-being and safety of both our service users and employees.
