Full Job Description

We are eager to recruit a Mental Health Practitioner who is passionate and enthusiastic about the health of homeless people and asylum seekers. We are looking for a professionally qualified RMN, OT or Social Worker who is suitably experienced to join The Homeless & New Arrivals Health Team. The team works collaboratively to develop and maintain a pro-active and responsive service aimed at meeting the social and health needs of homeless people, asylum seekers and refugees who have mental health and emotional problems. To work as part of a specialist multi-disciplinary health team within primary care services and provide holistic mental health assessments, effective interventions, and liaison with relevant agencies. To develop excellent working relationships with other providers for this client group and facilitate a service wide response where necessary.,

  • To develop the mental health provision for homeless people asylum seekers, refugees and new arrivals to Bradford in partnership with colleagues from the Homeless and New Arrivals Health Team.
  • To provide mental health assessment, triage and support for clients referred to the team, demonstrating awareness of individual need and lifestyle. To provide brief, effective evidence based interventions in a variety of settings. To manage the allocation of referrals ensuring signposting is as transparent as possible from the client's perspective. To coordinate the next steps to care e.g. to primary care, specialist services, or secondary care etc. To liaise closely with primary care mental health services to ensure the needs of the client group are met. To work in partnership with secondary care, specialist teams, primary care organisations, statutory and non-statutory providers to develop and agree referral protocols and pathways to ensure smooth transition and better outcomes for the service user.
