Full Job Description

An exciting opportunity has arisen in the Mental Health Advice, Support and Access team her in Just One Number. We are looking for a Band 5 nurse or allied health professional, to join our dynamic and innovative team - 0.8 WTE (30hrs)
We triage all non-emergency mental health requests for support for 0> 25yrs, in Norfolk and Waveney. Identifying the best pathways and sources of support.
We are looking for an individual who is flexible and adaptable, friendly and able to engage with families and young people to understand their needs and how best to meet them. This is an office-based role, where you will work directly with peers and other mental health professionals. We will be looking to extend our offers to provide single session interventions and continue to build on our knowledge and network of Mental Health system providers. For the right candidate, we will provide preceptorship and support. It is a rewarding role in a developing service that is making a difference to the mental health of children and young people.

To have clinical expertise in the mental and emotional health needs of children, young people, and their families, (covering 0-25 years of age) and be able to elicit information from professionals, carers and young people to support full triage via telephone and other forms of digital technology which enable timely clinical decisions to be made.

To have a robust understanding of local emotional health providers and be able to give children, young people and their families and referrers appropriate advice and guidance and as needed make appropriate referrals and support colleagues to make referrals to the appropriate service.

To have knowledge of child and young people's mental health needs. This will involve information gathering, analysing information including providing risk management and safety planning, with children, young people, parents, teachers and being an active link for other stakeholders such as schools, GPs, mental health, and voluntary sector organisations.

To develop skills and confidence to support colleagues within the wider Just One Number service, especially in understanding of current mental health needs of children and young people, and to provide information on managing risk and care planning.

1. To develop skills to contribute to production and maintenance of digital resources on the Just One Norfolk websites and promote of these within JON and with other stakeholders - allowing the identification of appropriate use of self-help material and understanding when further clinical assessment is indicated.

2. Use Data to support measures to monitor effectiveness and establish outcomes and share these within JON, CCS and NSFT.

3. Uphold the professional code of conduct for the Nurse, Midwife and Health Visitor, including maintaining confidentiality and maintaining registration in line with the NMC or appropriate governing body. Or other relevant professional councils dependent on core qualifications such a allied health care professionals - OT. SW

4. Under the Supervision of a specialist practitioner supporting clinical decisions and demonstrate a high level of clinical understanding. Follow the robust clinical risk management processes that are in place and contribute to ensuring they are adhered to within JON.

5. Demonstrate that evidence is translated into sound clinical practice.

6. To maintain a clinical profile being visible, accessible and approachable to staff, stakeholders and the public. To ensure that open and honest communication channels are sustained.

7. Support staff within Just One Number and wider services to fully understand and use agreed clinical pathways, system agreements and standard operating procedures to ensure the most appropriate and safest outcome for the CYP.

8. Ensure the voice of the child and young person is pivotal in all aspects of clinical practice and development.

9. Act as a mentor to students in line with NMC requirements for Registered Nurses including attending annual mentor updates and evidence for Triennial Review, or in line with other allied health care professionals as required

10. Participate in recruitment, selection, and induction of junior staff.

Communication

1. Reports and is accountable directly to Team Lead and Clinical Lead for mental health.

2. To establish positive and effective internal and external working relationships.

3. The post holder is required to utilise a wide range of communication skills and will be required to sensitively communicate on complex issues to a wide range of audiences, including children, young people, parents and schools and other organisations, in order to inform and influence clinical decisions.

4. Use sophisticated communication skills to impart difficult or unwelcome news to patients and staff for example complaints, patients experience,

5. To maintain necessary communications and liaison with all members of the Just One Number Team.

6. To maintain regular contact with the Line Manager and participate in clinical supervision, line management and other relevant meetings

People Management

1. Development of junior staff

2. Maintaining own professional development and requirement to take part in appraisal and KSF process

Clinical and Practice Governance

1. Observe and maintain strict confidentiality with regards to any patient/family/staff/records and information in line with the requirements of the Data Protection Act.

2. Any data that is taken/shared as part of a phone call or transferred electronically must be undertaken with regard to the Trust Information Governance and Information Security policies.

3. The post holder must adhere to the Trust risk assessment and risk management processes

4. The post holder must adhere to infection control policies and procedures

5. Undertake mandatory training and any other training relevant to the role as required by Cambridgeshire Community Services NHS Trust.

6. The post holder must participate in clinical and other audits as required

7. Provide patients and their families /carers with information on standards they should expect from the team.

8. Participate in clinical supervision on a regular basis

9. The post holder is required to participate in relevant emergency preparedness process for their team.
