We would like to recruit highly motivated band 6 Midwives to lead the Young Parent Maternity Service within our welcoming, supportive and friendly Enhanced Support Midwifery team . This will be involve working across central Lancashire with young people and their families. The role is a fixed term contract for a period of twelve months.

You will provide antenatal and postnatal continuity of carer to young people and act as an advocate for young people and their families to coordinate care based on their individual needs optimising mental as well as physical health.

You will work in partnership with a multi-agency and multi-disciplinary team from midwifery, health and social care to provide and co-ordinate the highest standards of care for young people and their families some of whom may have complex social issues., ? You will perform a key role within the enhanced support midwifery team. Acting as the link between the maternity service and other healthcare, educational and support services involved in the young person's care planning.

? You will work closely with the enhanced support midwifery team to ensure maternity care is responsive to the young person's needs.

? You will work autonomously, being responsible for managing a caseload and ensure young people are referred appropriately and properly safeguarded.

? You will have a sound based knowledge and act as a resource to colleagues to enable them to provide holistic care that optimises the health and wellbeing of young people and their families with/without complex social needs including mental health issues.
