We are looking to recruit a Registered Nurse to work 2 nights per week (22.00hrs 07.00hrs) including weekends and Bank Holidays. If you are passionate about palliative care and want to be part of a supportive team that makes a real difference within the community, then this could be the perfect opportunity for you.Main duties of the job We provide nursing care to patients who require symptom management and end of life care at home. The post holder will have an important part to play in providing holistic care and meeting the specific needs of these patients, their families and those important to them without direct supervision. This will include:

• Assessment of needs and symptoms and implementation of individualised care.
• Giving appropriate information to their families and involve them in developing the plan of care and its ongoing evaluation.
• Administering prescribed medication, as required.
• Ensuring accurate records are maintained.
• Ensuring co-ordinated team approach by use of effective communication within the team and other community services involved.
• Giving sensitive information and support to those closest to the patient.
