Full Job Description

Single Point of Access are looking for an experienced nurse who is highly motivated and has excellent communication skills, to join our team based in Ellesmere Port. Working as a Nurse Coordinator, the post holder will effectively manage referrals from a variety of care settings and utilise various care pathways, ensuring that patients are referred to the most appropriate service for treatment., The post holder will be responsible for facilitating the management of patients with unplanned care needs by ensuring that patients are referred to the most appropriate service which meets their individual health needs, adopting a patient centred approach. This includes admission to hospital where appropriate, preventing unnecessary hospital admissions by utilising community services and facilitating a smooth transition of care between primary care, secondary care and community care settings.

This is a non-patient facing role working within a call centre setting. There are 2 part time posts - 30 hours per week and 15 hours per week. Hybrid working available.
