We are part of a large integrated drug and alcohol service working with some of Cornwall's most excluded and disadvantaged people. Successful candidates will work as part of the adult community team, alongside in-house services which include outreach, hospital, clinical, criminal justice and a residential rehabilitation facility. We have an exciting opportunity in our Cornwall services for a full-time Outreach Worker to join our busy and dynamic Hospital Outreach Service based in Truro. This is an exciting and challenging role that works from a
trauma informed perspective based in the community covering all of Cornwall. Due to the outreach basis of this role, a full driver's licence and access to a car is required. The Hospital Outreach Service provides a rapid response to individuals who are frequent attendees at the Royal Cornwall Hospital in Truro and have issues related to substance use. The team works within a multi-disciplinary model of practice with partners in safeguarding, housing and homelessness services, domestic abuse
