Full Job Description
The post holder will work as part of Team of Band 7 nurses with responsibility for the care and management of children with chronic conditions, and will act as Clinical Nurse Specialist for children and young people with Allergy, Asthma, Cystic Fibrosis and other complex respiratory conditions within this team., To provide assessment, training, support and guidance to families at point of diagnosis and following acute episodes.
To work with Lead Consultant, Matrons, Management and commissioners to develop services for Children and Young People with Allergy, Asthma, CF and other respiratory conditions.
To lead on discharge planning process for children with complex conditions, supporting the ward team with the liaison process between Community and commissioning services.
To develop and deliver Nurse Led Clinics for children and young people with Allergy and Respiratory conditions in collaboration with the Lead Consultant.
To constantly evaluate the views of the young people and their carers' in order to oversee improvements and make changes to service delivery.
To educate and train professionals on the Bedford Site about the care and needs of young people with relevant long term conditions and in doing so improve the experiences of all young people in the Trust.
To work confidently in an acute paediatric setting, contributing to the Senior Nursing rota, giving clear leadership and clinical guidance.
Work closely with schools and or colleges to set up unique care plan for each of these complex children and possible provide virtual training for key members of staff to ensure patient safety.
