Full Job Description

A Vacancy at University Hospitals Bristol and Weston NHS Foundation Trust. The post holder will work as an integral part of the multidisciplinary palliative care team to provides expert advice and management for patients with life limiting illnesses, to enable timely and effective care. This includes those with malignant and non -malignant diagnoses. They will demonstrate a high level of expertise in within the specialist field, can work independently and within a small team, and demonstrate skills in teaching, organisation, audit and service development. They will provide support and education for colleagues and work in close collaboration with multidisciplinary teams within the Trust and community to ensure patients receive the highest standard of clinical care and a seamless service.   • Demonstrate expert knowledge and skills across a range of procedures and practices. Act as a resource for patients, carers and staff.
  • Be responsible for the assessment, planning implementation and evaluation of specialist individualised management plans
  • (With support) work autonomously, demonstrate high levels of expertise and initiative within complex situations involving multiple teams
  • Work closely with patients' and their families when communicating significant news and having discussions around prognosis and future care options.
  • Provide advice on complex pain, symptom management and end of life care.
  • Work with the team on projects aiming to improve care of palliative and those at the end of life across the Trust., children, young people and vulnerable adults.
