Full Job Description
This exciting role will be to work in partnership with people with Parkinson's, their families/carers and other professionals, providing specialist, evidence based and high-quality care. The post will involve, leading, organising and delivering ongoing education for patients and training for professionals, carers and contribute in the development of local Parkinson's care strategies and guidelines.
The Parkinson's team covers the whole of Morecambe Bay Footprint therefore flexibility will be required across 3 main hospital sites.
Main duties of the job
The successful post holder will be required to work as part of the integrated team and will get the opportunity to further develop their skills across all areas. Fundamentally the nurse will be required to;
Assess people with Parkinson's
Agree and review the treatment plan for inpatients and deliver nurse led clinics
Work closely with multi-disciplinary teams in the hospital and community setting
Provide support and advise to healthcare professionals on quality evidence based care as per guidelines
Provide structured education groups providing knowledge and skills to people with Parkinson's to support their self-management
Deliver formal and informal education to health professionals
Undertake evaluation and clinical audit to improve patient care and clinical outcomes
Support an integrated nursing team and work across primary and secondary care settings as required for patient care
Applicants will need to demonstrate at least 2 years recent and significant experience to be a driving force within the team.
We operatefrom three main hospitals-Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, andUlverstonCommunity Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies,daycase and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
Our community services for adults are provided in people's homes, community centres, clinics, GP Practices, community hospitals and our main hospitals. Our aim is to work with people to help them remain independent, improve their health and manage their conditions through high quality care, advice and support.
