PCN Care Co-ordinator

East Bedford Primary Care Network have an exciting opportunity for a hardworking care co-ordinator to join our primary care network on a full-time basis, however part-time can be considered if over 24 hours per week. The role covers the following GP practices in Bedford;

• Putnoe Medical Centre
• Linden Road Surgery
• London Road Health Centre
• Cauldwell Medical Centre
The post-holder will help us to improve the care provided to some of our most vulnerable patients. If you are a skilled care co-ordinator, with excellent person-centred planning experience and strong local knowledge for accessing key support, then we would love to hear from you.,
• Working with GPs and other professionals within the four practices which form the PCN to identify and manage a caseload of patients.
• Conducting health reviews of patients with dementia, mental illness or learning disability.
• Using knowledge of locally available support and services to develop care plans with patient, focusing on prevention.
• Supporting the coordination of all key activity, including access to services, advice and information.
• Using clinical systems on a daily basis, with confidence to run searches and keep care records up-to-date.
