Summary

• Preferred days/times: 6 hours per day, Friday and Saturday


• Tasks/duties: To support with step transfers for toilet routines, support with meal and drink preparation, and with going out into the community. The client's mobility vehicle will be used for travel.


• Client main diagnosis: A fall in 1998 resulted in a fracture of lower back, nerve damage, chronic pain, general weakness and reduced mobility. Irritable bowel syndrome. Stage 4 Terminal cancer - short prognosis.


• Manual Handling: Step transfers must be supervised.


• Communication: No communication barriers.


• Client enjoys: Arts and Crafts, Socialising.
