Full Job Description

Service users with severe and enduring mental illness are known to have poorer health outcomes than the general population. For this reason, the North London Mental Health Partnership is providing support to service users around their physical health needs. The post holder will work together with team members conducting supervised specialist assessments of service users with a special interest in their Physical health. They will lead on ensuring that service users with enduring mental health issues are not discriminated by virtue of their mental illness in terms of their physical health. The post holder will lead on supporting service users with blood tests, BMI, ECGs, BM, maintaining Lithium registers, and other physical health checks as required. · Have informed knowledge and understanding in the psychological, social, physical and spiritual needs of all service users. · Actively participate in the care and rehabilitation of service users as per the plan of care determined by registered practitioners. · Maintain clear, timely, comprehensive, and contemporaneous written clinical records on all patients in accordance with Trust policies. · Utilising IT skills and knowledge to access and input clinical information and data to the Trust Electronic System (RiO) and Local Authority IT system. · Undertake a range of duties which involve direct patient care, under supervision of the physical health lead clinician or other senior clinicians in the team. · Assist in the coordination of care delivery for a case load of service users in the community with mental health problems including those with severe and enduring mental illness under the supervision of a registered professionals, in close collaboration with the Directorate, psychiatric multidisciplinary team, service users and other community agencies.   • · Using the Dialogue plus care plan assess service user's health and social care needs. · Contribute towards the delivery of care using the recovery approach to provide therapeutic interventions under supervision by means of assessment and ongoing treatment in various settings. Design care programmes in line with risk assessment and psychosocial needs. · Utilise designated recovery tool with outcomes focusing on care planning, ensuring clients understand they will be worked with in a collaborative manner to engage in social inclusion activities such as seeking employment, education, and independent living. · Act in accordance with the direction of an appropriate practitioner, for example nurse, doctor or pharmacist, to administer medication as and when required following competency assessment and supervision. · To be conversant with factors that cause mental illness and preferred treatment methods. · Support the assessment and management of patient care needs through initial data collection, monitoring of patients' progress, feedback, discussion, and reports and in care reviews with the clinical team. · Assist in the physical health care assessment of service users and report any changes in service user's physical health to the registered practitioner. · Provide information of health promotion and healthy living either individually or in a group setting. · Provide audit information around physical health as required. · Be a qualified Phlebotomist and able to take blood from service users or be willing to be trained. · Carry out ECGs on service users or willing to be trained to do so. · Provide patients and their carers with a service that is culturally sensitive and gives due consideration to any special needs. · Have informed knowledge and understanding of the Mental Health Act 1983 amended 2007 in particular Part 3 of the Act and other legislations such as Safe Guiding Children, Adults, DOLS, Mental Capacity Act 2005 and others relating to care of service users in the community. · To undertake and assist in the organisation of the day-to-day smooth running of department/ unit/ service. · Contributes towards the ongoing quality of service and care and identifies any risk issues in relation to the patient's/ client's health and social care.
