Practice Nurse

1) Providing assessment, screening and treatment services that are relevant to patients and their families, with emphasis on improving diet and exercise routines through health promotion clinics. Referral to GP and other agencies as practice policy dictates. 2) Developing and organising, with others, nurse-led clinics for the chronic disease management programmes within the practice, specifically: Coronary heart disease Chronic obstructive pulmonary disease Asthma Diabetes Heart failure Peripheral Vascular disease Hypertension Stroke/TIA 3) Responsible, with other members of our nursing team, for the development, organisation and running of routine clinics specifically: Immunisations Travel health and vaccination programmes Cervical smears Contraception advice 4) Develop, organise and run the Practice immunisation programme. 5) Assess risk factors leading to ill health and advise and assist in suitable interventions. 7) Perform various clinical procedures such as ECG recordings,
venepuncture, cervical smears, routine blood pressure monitoring, intramuscular and subcutaneous injections. 8) Devise self-management plans for patients with Asthma. Administrative 1) Develop practice protocols and guidelines in conjunction with GPs. 2) Provide clinical advice and mentoring to less experienced clinical staff.
