Full Job Description

Birtle View Medical Practice in Heywood is looking for a Practice Nurse to join our friendly team as we look to better serve the needs of our patients. You will be supporting patients to be healthy by monitoring long term conditions, and by focusing on health prevention and screening. You will be working with a team of GPs, a Nursing team and an excellent admin/reception team that keep the show on the road. There will be professional development opportunities, including cross-practice meetings with other Practice Nurses and opportunities for further training., The post holder will be responsible for the delivery of Practice Nursing services, alongside another Practice Nurse, to the practice population. Supported by the Practice team, they will deliver care and focus on supporting patients to be healthy, monitor long-term conditions, health prevention and screening activities. They will work collaboratively with the General Practice team to meet the needs of the patients, supporting the delivery of policy and procedures.

Birtle View Medical Practice is ran by Hope Citadel Healthcare, along with 9 other practices in the Greater Manchester area.

Hope Citadel Healthcare is a not-for-profit community interest company, who have been commissioned by the local ICB to provide NHS services to the local area where the practices are located.

£39,000 to £47,000 a year dependent on experience and qualifications such as prescribing
